The examinee or proctor wants to formally request a review of a decision or incident related to the ServSafe course or exam.

Request Submission:

To request a review by the governing committee, the examinee or proctor must submit this form and supporting documentation directly to National Restaurant Association Solutions (NRA Solutions).

Next Steps:

The requestor will receive an email with the final decision within 30 days after the Association receives the completed form.
