ServSafe Name Correction Form

Purpose

Use This Form If:
  • The examinee’s name was misspelled on the answer sheet • The examinee has had a legal name change after the exam date.

  • The online exam was taken under the wrong account and the examinee’s name is incorrectly reflected on the certificate.

To maintain the integrity of exam records, all information submitted must be accurate and truthful. Submitting false or misleading information may result in the rejection of the correction request and could affect exam certification status

Request Submission:

To request a name correction, the examinee or representative must submit this form and supporting documentation directly to National Restaurant Association Solutions (NRA Solutions).
