ServSafe Request for Exam Accommodation

Purpose

Use This Form If:

The examinee wants to request support or accommodation considerations during the exam.

Request Submission:

To request exam accommodation, the examinee or representative must submit this form and supporting documentation directly to National Restaurant Association Solutions (NRA Solutions). This form should be submitted 15 days prior to the scheduled exam date.

Next Steps:

There will be a notification email sent whether the request is approved or denied. If approved, the examinee must inform the proctor so appropriate preparations can be made.
